Due to campus closures, students must submit their Academic Progress Report online. Once campus is open, we may begin accepting a physical copy of the form. For assistance in completing the online form, please refer to the Instructions to Submit the APR Online (PDF).

Students with a “D” grade or lower will be contacted by a staff member to set up an appointment with a NextUP/Guardian Scholars Counselor.

Forms must be submitted no later than 4:00 pm on Thursday, April 30, 2020 in order to remain eligible to fulfill Second Contact. Your Second Contact is not complete until a staff member approves this form. Incomplete forms will not be accepted. 

There are no exceptions to the 4:00 pm April 30, 2020 deadline.
