Students must complete the 1st Contact Online Form by submitting an updated Student Educational Plan (SEP), a current class schedule, and a time management sheet.

The purpose of submitting your SEP and class schedule and a time management sheet is to help us better understand how you are managing your time for your classes and study time.

The 1st Contact Online Form will be posted on this page during open submission timeframes. This current timeframe will open on September 28, 2022 and close on October 7, 2022 at 11:59 pm.
